OFCCP Issues New Self-Identification of Disability Form
On April 25, 2023, the Office of Management and Budget (OMB) approved OFCCP’s revised Voluntary Self-Identification of Disability Form (CC-305). OFCCP revised the form to update the preferred language for disabilities and to include additional examples of disabilities.
If you are a covered contractor, generally those required to maintain written affirmative action plans under EO 11246, you must implement the new form by July 25, 2023. You can download the new form here.
Note: The voluntary self-identification form is an OMB‐approved form. The only portion that contractors may modify or delete is the “For Employer Use Only” section, which was added to give contractors flexibility to enhance their recordkeeping for data analysis required by OFCCP’s regulations.
